The Challenge

Records for this programme were fragmented across multiple departments with no single system of record.

Different agencies held different pieces of the same case, with no visibility across organisational boundaries.

Inter-agency referrals and updates happened over phone and email — creating gaps in the record that were difficult to reconstruct later. Case workers couldn't see the full history of a case before making decisions. Duplication of effort was common.

When oversight bodies requested documentation, the manual process of gathering records from multiple agencies was time-consuming and produced inconsistent results. The compliance risk was significant.

Our Solution

We built a confidential multi-agency records management system with a clearly defined case lifecycle — from structured intake through active case management to resolution.

Each case carries a complete record that follows it across agency boundaries. Role-based access ensures each agency sees only what their mandate requires, while authorised staff can see the full case history.

Intake is structured to capture all required fields at the point of creation, eliminating incomplete records from the start.

Inter-agency coordination happens inside the system — referrals, status updates, and case notes are tracked as part of the record rather than lost in email threads.
